Job Description

Under the direction of the Clinical Director the Case Manager (Assistant to the Benefits Specialist) will provide screening, application and enrollment assistance and education regarding public benefit services to individuals living with HIV/AIDS at multiple sites in South Los Angeles.

Required Qualifications

EDUCATION:
• Regionally accredited associate’s degree required; and/or equivalent of combine education and experience in the related field.
• Bachelor’s degree is preferred.

EXPERIENCE:
• 1-2 years of experience working with gay/lesbian/bisexual/transgender individuals, individuals living with HIV/AIDS; homeless individuals; individuals who are dual diagnosed.
• Knowledge of terminology specific to HIV/AIDS is required.
• Knowledge of specialized government programs such as Section 8, Social Security, General Relief, and HOPWA and their eligibility requirements in regard to persons with HIV is essential.

Minimum Requirements

KNOWLEDGE/ABILITY/SKILLS:
• Ability to write and prepare statistical and narrative reports and professional correspondence.
• Computer literacy in Microsoft Office (Word, Excel, Outlook).
• Interpersonal skills to establish and maintain effective working relationships with clients, Benefits Specialist and other agency staff.

Preferred Qualifications

Bachelor’s degree is preferred.

Working knowledge of other community resources and services/disciplines provided by other programs.

Essential Duties

• Conduct brief screenings for benefit program eligibility to a minimum of 213 clients each year.
• Assist clients with benefits applications and enrollment when indicated by screening process.
• Provide clients with referrals for Case Management, Mental Health and other services based on assessment.
• Refer clients for appeals assistance.
• Document all services provided according to standards set forth in County of Los Angeles – Division of HIV and STD Programs(DHSP)funding contracts.
• Work with Clinical Director and Benefits Specialist to ensure that the Benefits Specialty services are easily available to individuals living with HIV/AIDS in South Los Angeles.
• Attend trainings provided by agency and/or DHSP pertaining to the Benefits Specialty Program.
• Attend agency staff meetings, in-service training seminars, other agency meetings, and other trainings/conferences identified by the Clinical Director.
• Enter monthly service data in Casewatch in a timely manner and prepare other demographic and statistical reports as required by funding agency and Clinical Director.
• Collaborate with multidisciplinary team of physicians, mental health clinicians, case managers, treatment educators, and peer support advocates with regard to public benefit eligibility program.
• Perform other related duties as assigned or requested.

The ability to perform highly detailed work with sustained attention and care while providing/obtaining information on numerous inquiries is paramount to success in this position.
